Anyway, I'd like to know the time and depth of move 6. Seems like a major meltdown to me.

1. The base of games in the book were selected by Rybka
2. Unfortunately the first probe into the game happens at move 12 and this was move 6
3. The minimum number of games required for the move to be played from the book was zero, as the games are all pre-checked, this should be safe-ish. This Ng4 comes from a dumbo game between BBchess and Typhoon from CCRL, although where the moves come from shouldn't matter as Rybka checks all the games
There are a couple of fixes to this:
1. Add an initial game probe earlier than move 12, I'll probably do this anyway
2. Run with min-game 2, this is almost definitely what I'll do, this would have fixed the last draw also and given us more of a chance at 14/14
The trouble with min-game 2 of course is that it could lead to missing strong single-game novelties, min-game 2 (rather than min-game 0) changes the size of the Polyglot book from 240mb to 15mb ! I guess I'll just deal with those really strong single games in an overlay book.
Anyway, you'll be happy to know that Ng4 is no longer in the book, and Rybka was fortunately strong enough to win the game anyway.
PS. What is your view on 1 ... Nf6 in reply to 1. b3 in general?
I'm relieved to find out that Rybka didn't pick this bizarre move. I am always finding trashy moves like this in my book, but I assumed your book was better vetted. :-)
I've been using 1...Nf6 as my reply to 1. b3 for a while, mainly to try to prevent transposing into lines that I want to save for freestyle and correspondence games. Its played less than 10% of the time and in engine-engine games, black frequently follows with ...d5 and ...c5 transposing into much more common lines. Some of these lines lead to positions that Chessbase categorizes as A08 which Rybka 2.3.2a plays very poorly (you should see these in your database). White has to be careful in these lines not to rush c4 (as your opponent did in the tournament) hoping for e5 (but we already know that, right?).

Thanks for your thoughts on 1 ... Nf6, I know you like zappa in these 1.b3 openings, I'm often amazed at how bad the eval is for white for such a long time, and then the true power of the white position is exposed moves 15 onwards. A good opening for Freestyles perhaps, at least in the qualifiers, to catch the engine jockey unware. Lately, in tuning this book in fact, I've seen a lot of games where white plays e3 rather than c4 and just gets rolled, so I'm not sure how early is early for c4.
As far as getting rolled without an early c4, probably we should arrange a time to hash this out on the Playchess server :-). Against ...d5 ...c5 ...Nf6 I've had good luck with e3 or f4. I usually get away with g3 too, but it isn't really sound. With an early c4, black can frequently get immediate equalization or better by pushing to d4 (Zappa is more likely to do this than Rybka 2.3.2a).
